Need an appointment?
Send a request through Weave. Our office will contact you to arrange a visit; submitting the form does not book one.

Complete your health history before your visit. Call the front desk if you need help with the form.
Health history formIt helps to have recent dental images and treatment notes available for the consultation. The front desk can help arrange records from your dentist.
- Medical history and current medications
- Dental insurance details, if you have coverage
- Recent radiographs or CBCT from your dentist
- Any treatment plan you would like a second opinion on
Here are questions you may want to bring to your consultation.
What to expect
What happens at your first visit.
- 01
Before you arrive
We’ll ask for your medical history and any recent radiographs from your dentist. The front desk can help you prepare and arrange records.
- 02
The examination
A full periodontal charting, an assessment of bone and soft tissue, and imaging where it’s warranted. Expect around an hour.
- 03
The conversation
We put your own scans on the screen and explain what you’re looking at. We discuss the diagnosis and answer your questions.
- 04
The plan
A written plan with sequence, timing and cost. We send it to you, and to your own dentist if you have one. Nothing is scheduled at the first visit unless you want it to be.
Insurance & payment
Costs and coverage.
We review the plan and costs with you after the examination, before treatment is scheduled.
-
Estimates in writing
Every plan comes with a written breakdown of sequence, timing and cost before anything is booked.
-
Insurance
Both doctors participate in Delta Dental Premier. We can help you understand your benefits, request a pre-treatment estimate, and submit a claim. Coverage and costs depend on your plan.
-
Payment & financing
Cash, check and credit card, with financing through Cherry. We arrange the financing before treatment begins, so you know the full cost going in.

New patient FAQ
Practical questions.
Do I need a referral to be seen?
No. You can book directly or come through your dentist. We also see patients looking for a second opinion.
What should I bring to my first visit?
A list of your current medications, your medical history, your dental insurance card if you have coverage, and any recent radiographs or CBCT from your dentist. The front desk can help arrange records from your dentist.
Do you take my insurance?
Both doctors participate in Delta Dental Premier. We can help you understand your benefits, request a pre-treatment estimate, and submit a claim. Coverage and costs depend on your plan.
How much will treatment cost?
We provide a written estimate after the examination and before scheduling treatment. We accept cash, check, and credit cards. Cherry financing is available.
Where do I park?
Validated parking in our building is $1.50 for two hours. Metered parking is also available on South Beverly Drive. We’re between Olympic and Gregory Way, directly across from Pavilions.
I’m anxious about dental treatment. What are my options?
Say so when you call. It changes how we schedule you and how we talk you through the visit. Both doctors hold a California moderate sedation permit and offer oral and intravenous sedation, and for the cases that warrant it we’re equipped for general anesthesia in our own operatory with an anesthesiologist present. A consultation itself involves no treatment at all.
