This policy covers information collected through beverlyhillsperio.com. Our Notice of Privacy Practices explains your rights concerning your patient record and other protected health information.
Sending health information? Use the Weave form on our contact page to request a consultation. If you already have an appointment, use the health-history form on the New Patients page. Referring offices can send records through Referral Intel. Call us for urgent help.
Information we collect
Information you give us
Weave hosts our consultation-request and health-history forms. It receives what you enter and makes the submissions available to the practice. See Weave's privacy policy.
Referral Intel hosts the referral form. It receives the referring office's contact information, patient details, reason for referral, provider preference, and any uploaded records. See Referral Intel's privacy notice.
Information collected automatically
Our website host may log your IP address, browser details, and the pages and times you visit.
What we do not collect
- No analytics or advertising trackers. We do not use analytics, advertising pixels, heatmaps, session recording, or a third-party tag manager.
- No advertising cookies. Our own pages do not set cookies for advertising or cross-site tracking.
- No sale of personal information. We do not sell personal information or share it for cross-context behavioral advertising.

A consultation request does not add you to a mailing list.
Who else sees it
These services handle information when you use their features:
- Website host. Serves the site and may keep technical logs.
- Weave. Receives consultation requests and health histories through its forms. Its form pages may use cookies and process technical information.
- Referral Intel. Receives referral information and uploaded files. Its form uses Cloudflare Turnstile to prevent abuse.
- Email. Our email provider carries messages sent to the front desk.
- Google Maps. Loads only when you select “View map.” Google may then receive your IP address and set cookies.
- TikTok. Videos load only when you choose to play one. TikTok may then receive technical information and use cookies or similar technologies.

Cookies
Our host may set a security cookie. Weave and Referral Intel forms may use cookies; Google Maps and TikTok may do so when you choose to load them. You can manage cookies in your browser. Blocking them may affect the forms. If a form does not work, please call us.
